Additional Documentation Requirements

In order to verify that at least 80% of funds raised by your organization are being directly allocated to dog rescue operations, we require the following documentation to be submitted with your application:

(You can upload a file here in this form or you can send the files/documents to [email protected])

1. Evidence of Fund Allocation (Minimum 80% Direct to Dogs)

2. IRS Form 990 (Tax Filing)

To verify your organization's financial standing and transparency, please submit a copy of your most recent IRS Form 990. This form will provide an overview of your organization's income, expenses, and how funds are allocated. This will help us confirm that the majority of your revenue is being used for the direct care of the dogs.

3. Veterinary References

Please provide at least two veterinary references from the veterinary offices or clinics with which you work. These references should verify the treatments and medical care your organization provides to rescued dogs.

The references should include:

1. The name and contact information of the veterinary office.

2. A brief description of the services provided to your dogs (e.g., medical care, vaccinations, surgeries, etc.).

3. The frequency with which your organization brings dogs to their clinic for treatment.

4. A statement confirming that these services are provided to the rescued dogs with proper care and attention.

These references help ensure that the animals in your care are receiving proper medical treatment and support.
